The large randomized TESTS trial examined thymosin alpha-1 as an added treatment in adults with sepsis. It did not significantly reduce 28-day mortality compared with placebo. Immune-related mechanisms therefore should not be simplified into a promise of stronger immunity or infection prevention; results depend on the disease and clinical setting.
